I almost bought my dad a massage gun for his birthday.
Then I remembered his pacemaker, and I stopped.
That pause is really why I’m writing this. A TENS or EMS massager — the kind with a small control unit and pads, or a wrap that goes around the wrist or knee — is a popular gift for parents with stiff shoulders or achy knees. It’s affordable, it’s compact, and it feels thoughtful. But before you add one to the cart, there are two decisions to make: which form factor fits your parent’s daily life, and whether the device is even safe for them to use at all.
This guide walks through both, using only what’s backed by clinical sources — not the glowing claims on a product listing page.

What These Devices Actually Do
TENS (Transcutaneous Electrical Nerve Stimulation) and EMS (Electrical Muscle Stimulation) both send a mild electrical current through the skin, but they’re not doing the same job.
TENS works on nerves. It’s meant to interrupt or change how pain signals reach the brain, and many clinicians believe it also nudges the body into releasing its own natural pain-relieving chemicals. It’s used clinically for things like osteoarthritis, tendinitis, and fibromyalgia.
EMS works on muscles. It causes them to contract, which is more about circulation and muscle recovery than pain signaling.
Here’s the part I didn’t expect: TENS units are FDA-cleared Class II medical devices. Many of the “EMS massagers” sold online are marketed as general wellness or consumer products instead, though a few foot and leg models do carry their own FDA clearance. If pain relief is the actual goal, that clearance is worth checking before you buy.
Does It Actually Work?
I want to be honest here, because most product pages aren’t.
A Cochrane overview of TENS research for chronic pain could not confidently say whether TENS relieves pain better than a sham (fake) device. The underlying studies were rated very low quality, often with small numbers of participants, and there wasn’t reliable evidence that changing the frequency, intensity, or electrode placement made a meaningful difference.
That doesn’t mean it’s useless. It means the evidence just isn’t strong enough to call it proven. The encouraging side of that same research is that no serious adverse events turned up across the trials for people who were eligible to use it — the open question is effectiveness, not major safety risk.
So the honest way to frame this gift: it’s an adjunctive comfort tool, something that might help your parent feel a little better day to day. It is not a cure, and it’s not a replacement for actual medical care of an underlying condition.

Gel-Pad Type vs. Wrist/Knee Wearable — Which Fits Your Parent?
This is really where the gift-buying decision lives. Both types use the same underlying technology; they just deliver it differently.
Gel-Pad Type (Control Unit + Sticky Pads)
You place adhesive electrode pads directly on the sore spot — shoulder, back, calf, wherever — and control intensity from the unit or a small remote.
What’s good about it
– Goes almost anywhere on the body, so one device covers a lot of ground.
– Lower upfront cost, and the unit itself is small and quiet.
– Many models include a wireless remote and an auto shut-off around 30 minutes, which is a nice safety touch for an older user who might doze off mid-session.
What’s not so good
– The pads are consumables. They lose stickiness with repeated use — typically somewhere around 10 to 30 applications, depending on quality — and then need replacing. At one to two uses a day, a pad might last roughly three months. That’s a small but ongoing cost your parent may not think to budget for.
– Positioning the pads correctly takes a bit of care each time, which can be fiddly for someone with arthritis in their own hands.
– Skin irritation at the pad site is possible.
Wrist/Knee Joint-Specific Wearable (Brace or Band)
This wraps around the joint itself. Some versions still use gel pads, but others use reusable silver-conductive fabric or water-activated designs that skip pads entirely.
What’s good about it
– Much easier to put on — it’s a wrap, not a precise placement job. That matters if your parent doesn’t have the patience or dexterity for pad alignment.
– Targeted for a specific joint, which is exactly what most parents with knee or wrist discomfort actually want.
– The reusable or water-activated versions have a lower running cost over time, since there’s no pad to keep buying.
What’s not so good
– It only covers one joint. If your parent’s shoulder acts up too, this device won’t help there — you may need a second one.
– Fit matters more than you’d think. A wrap that’s too loose or too tight gives uneven stimulation and just feels annoying, so sizing is worth checking before you buy.
– Cleaning and care instructions vary by product, so read them.
My honest take: if your parent has pain in a few different spots — shoulders one week, lower back the next — the gel-pad type is more flexible. If it’s really about one joint, usually a knee, and your parent isn’t going to enjoy sticking pads on themselves every day, the wearable is the kinder gift.

The Safety Checklist You Cannot Skip
I’ll be blunt about this section, because it’s the one part of this guide that actually matters more than which type you buy.

Do not use without a doctor’s approval — treat as contraindicated:
– Pacemaker or implanted defibrillator (ICD). The electrical pulses can travel toward the device’s leads, and the device may misread that interference as a heartbeat. In the worst case, it can stop pacing correctly or, in a pacemaker-dependent patient, trigger a dangerous rhythm. Some clinical literature notes that a small number of pacemaker patients (not ICD patients) have used TENS safely under close medical supervision, with electrodes kept well away from the chest. For a home gift, though, that’s a call for a physician to make — not a consumer decision.
– Heart disease or arrhythmia. Talk to a doctor before use, and never place electrodes across the chest or near the heart.
– Pregnancy. Avoid use, especially on the abdomen or lower back.Use only with caution, and ideally after checking with a doctor:
– Epilepsy or a seizure disorder.
– Broken, irritated, or diseased skin at the intended site.
– Impaired sensation, such as diabetic neuropathy — if your parent can’t fully feel the stimulation, the risk of skin injury goes up.Usage rules, every time:

– Follow the manufacturer’s recommended session length and don’t exceed it.

Before you even wrap this gift, ask — or find out — whether your parent has a pacemaker, an ICD, or a diagnosed heart condition. It’s an awkward question to raise before a birthday, I know. It’s a much easier one than an emergency room visit.
So, Which Should You Buy?
If I’m summing this up in one line: buy for how your parent will actually use it, not for the feature list.
A parent who deals with soreness in different places benefits more from the flexibility of the gel-pad type, and won’t mind the small ongoing cost of new pads. A parent whose main complaint is one knee or one wrist, and who’d rather not fuss with sticky pads, will get more use out of a wearable wrap — especially a reusable or water-activated one.
Either way, this is a comfort device, not a treatment. If your parent has a real, persistent pain problem, a massager can sit alongside their actual care — it shouldn’t replace a conversation with their doctor.
